Requests for changes to UCPath configuration or department information may be made through the change request forms.
Change requests are typically submitted by a department head, an HR Business Partner or an Administrative Officer within the department.
Change Request Forms
- Departmental Change Requests
For any personnel realignment needed due to a department change, please work directly with your service channel
- Department Change: Use this request form to change a department name, change the main building location for a department, or inactivate a department.
- Department Realignment: Request to realign (move) department from one division to another division.
- New Department: Submit this form when requesting a new department to be created.
Davis campus: Please contact your funding authorizer to obtain the correct funding string. Look up your funding authorizer in Smartsheet.
UC Davis Health – When creating a new department, please ensure the appropriate Job Builder (JB) roles are established via the Request Form. Find your JB access role authorizer in this spreadsheet
- Add a New Division or Sub-division: This is a rare request as a result there is no submission form. Please submit a request to ucpath@ucdavis.edu and the support team will connect directly with you.
- Department Change: Use this request form to change a department name, change the main building location for a department, or inactivate a department.
- Building Location Change Request
Use this request to add, update, or delete building location.
